[Music Entry Form]

I know you don't want to read the next little bit BUT it sure makes life easier for both you and us. Many entry forms require us contacting someone or fixing a portion of the entry form and that requires a lot of time because of the volume of entries. Please carefully complete the forms.

NOTES REGARDING THE COMPLETION OF ENTRY FORMS
Please familiarize yourself with all the General Rules to avoid disqualification.

Incomplete or inaccurate entry forms may be returned to the applicant who will be responsible for returning them to the office by the closing date. Many entry forms are incomplete or inaccurate. This slows down the programming procedure.

Postal Code Information is part of your address and is necessary for mailing Program information to you. Please do not omit this information from your entry form.

All entry forms MUST be signed.

Each entry requires a separate entry form. In the case of duets, trios etc. please use one entry form only showing names of participants and accompanied with the full fee for this class.

All entry forms should show teacher's name, address and phone number. Most problems can more easily be solved by contacting the teacher rather than the student.

To avoid conflicts, please make sure you list your accompanist. This will aid in scheduling.

In order to speed sorting of entries it would be helpful to the committee if candidates would place the name of the category in which they are participating in the lower left hand corner of the envelope (example: Jr. Piano, Sr. Vocal, or Choir etc.)

Please list all possible conflicts on a separate piece of paper attached to your entry form (i.e P.D. days, religious restrictions, other musical commitments).

Requests for changes in repertoire will result in the entry being deemed non-competitive. Please complete your entry forms with thought and care.

Modern classes will refer to music published before 1950, and Contemporary classes will refer to music published after 1950. It will be the responsibility of the competitor to determine the publication date, and this should be included on the entry form.

There have been numerous complaints regarding own choice music in the Strings Classes. All own choice music MUST BE CHOSEN FROM the current Royal Conservatory, Toronto syllabus. Please enter the level at which you are currently studying

Kiwanis Music Festival, National Capital Region participants who are residents of Québec and study in Québec are now eligible to compete at the Provincial level in Québec

Flute ensembles now availabe in Woodwind Section.

If you are performing with a large group and your name is not listed, please inform us of this as it creates conflicts if you are performing in a solo category.

Official entry forms are available in the Festival Office. Send entries to Kiwanis Music and Dance Festival Association Kiwanis Music Festival
457 Edison Ave. Ottawa, Ontario K2A 1V1. Cheques must be made payable to the Kiwanis Music and Dance Festival Association. Your cancelled cheque is your receipt.
